Refer patients to Nutritionwize Diet Clinic for Registered Dietitian care, clinical nutrition programs, and other health services.

We accept referrals from physicians, nurse practitioners, specialists, allied health professionals, and community partners. Referrals can be sent using our referral form, or patients may contact us directly to book an appointment.

You can download our referral form and send completed referrals by fax. Our team will follow up with the patient to coordinate the appropriate service, location, and appointment type.
