It is a 5-session nutritional counseling education program delivered by a Registered Dietitian. This program is geared to provide you with evidence-based nutritional counseling that will help you self- manage your chronic disease in the area of nutrition. You can get trusted advice from a Registered Dietitian to help you make healthier food choices and may prevent the progression of chronic disease.
